Job Description Job Description Reports To: Assistant Director of RAP Housing Program: RAP/HUD Housing Location: Tarrytown, NY and Rockland, NY (mobile) Hours Per Week: 15-20 hours per week FLSA Status: Non-exempt , Part Time Annual Salary: $25 per/hr Summary Description : The Housing Case Manager plays a vital role in providing comprehensive support to individuals in securing and maintaining stable housing. This position involves collaborating with clients, community resources, and internal teams to address housing needs and promote overall well-being. Key Responsibilities : Conduct housing assessments and develop individualized housing plans for clients. Assist clients in accessing affordable housing options and navigating the application process. Collaborate with property management, landlords, and housing agencies to secure housing opportunities. Provide ongoing support to clients in maintaining stable housing and addressing any housing-related challenges. Facilitate housing-related workshops and educational sessions for clients. Coordinate with social service agencies, healthcare providers, and community resources to ensure holistic support for clients. Advocate on behalf of clients to address housing-related barriers and challenges. Maintain accurate and up-to-date documentation of client interactions, housing plans, and progress. Participate in interdisciplinary meetings and case conferences to discuss client needs and progress. Stay informed about housing regulations, policies, and resources to provide up-to-date information to clients. Empathy and cultural competence in working with diverse populations. Case management and organizational skills. Advocacy and negotiation abilities to address housing-related challenges. Collaboration and teamwork in a multidisciplinary environment. Crisis intervention and problem-solving skills. Key Performance Indicators (KPIs) : Successful placement and retention of clients in stable housing. Timely and accurate documentation of client interactions and housing plans. Positive outcomes in addressing housing-related challenges. Effective collaboration with internal teams and external resources. Requirements : Bachelor’s degree in Social Work, Psychology, or a related field preferred Minimum of 2 years of experience in housing case management or related roles. Knowledge of housing regulations, resources, and community services. Familiarity with electronic case management systems. Strong communication and interpersonal skills. Ability to work independently and as part of a collaborative team. Commitment to cultural competence and diversity. Possession and maintenance of a valid Driver's License with a satisfactory driving record according to our insurance carrier's standards. Must maintain current NYS Defensive Driving certification within 30 days of hire. Must maintain pre-employment clearances by applicable regulatory clearances as required. Legal eligibility to work in the United States without sponsorship. Special Considerations : Hours per week, reporting structure, and location are subject to change per program needs. Mandated Reporter: This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.
