Job Description:
Description Atlantic Health is seeking a dynamic and motivated
individual, to join the energetic and innovative team of healthcare
professionals in their Atlantic Visiting Nurse Division as a Per
Diem Physical Therapist to work 8AM-4PM including two weekends per
month, the Warren County area. The ideal candidate will evaluate
and treat patients in the patient's home and be a part of a
collaborative team of health providers. Atlantic Visiting Nurse
provides personalized care to patients in order to regain health,
stay out of the hospital, and remain living independently at home.
Whether or not it is managing life with a chronic illness, living
with a disability, returning home from the hospital or
rehabilitation facility, or need assistance with end-of-life care,
we are available to you in six counties throughout New Jersey:
Morris, Somerset, Essex, Sussex, Union, and Warren. Qualifications
Essential Functions/Responsibilities: Evaluate and treat patients
in their homes Minimum Education: High School Diploma or GED,
required Bachelor's in Physical Therapy, required Master or
Doctorate in Physical Therapy preferred Minimum Experience: 1 year
of experience in homecare, strongly preferred
License/Certification: NJ License in Physical Therapy, required
BLS, preferred Valid and unrestricted Driver License with a clean
driving record, required.
