caregivers who work together to provide the best care. Your Role In
the position of Physical Therapist (PT), you will assess the
physical capabilities of residents and create tailored treatment
plans aimed at helping them regain or maintain their mobility,
functionality, and independence. Your responsibilities will include
delivering hands-on therapy, educating patients and their families,
and collaborating with interdisciplinary teams to achieve the best
possible outcomes. Main Responsibilities Perform thorough
assessments of residents’ physical abilities and mobility using
standardized assessment tools. Create and execute evidence-based
treatment plans to enhance strength, balance, coordination, and
range of motion. Deliver therapeutic interventions, including
exercise regimens, gait training, and the use of assistive devices.
Provide education to residents, families, and staff on safe
mobility practices and home exercise routines. Accurately document
evaluations, progress notes, and discharge summaries in a timely
manner, adhering to regulatory and organizational standards. Work
collaboratively with occupational therapists, speech-language
pathologists, nurses, and other team members to ensure coordinated
care. Ensure compliance with all relevant state and federal
regulations. Qualifications Active licensure as a Physical
Therapist (PT) in the relevant state. Graduation from an accredited
Physical Therapy program. Preferred experience in long-term care,
skilled nursing, or rehabilitation settings. Excellent
communication, documentation, and organizational abilities. A
