seeking a full-time Nurse Practitioner or Physician Assistant for a
Hybrid – ED/Home-based (REACH - Resource for Elderly and Care at
Home) Program, delivering compassionate care to geriatric patients.

from you! Role: Open to Nurse Practitioners (FNP, ANP, AGNP) and
Physician Assistants (PA) Location: St. Vincent’s Medical Center
and Homecare based in the greater Bridgeport, CT area Setting:
Geriatric Medicine in the Emergency Department (ED) and home
setting, working within a multi-disciplinary care team Schedule:

Assistants Role Scope: Under established guidelines and in
collaboration and/or supervision with the practice physician(s),
Nurse Practitioners and Physician Assistants perform routine
physical examinations, and formulate medical, educational, and
psychosocial plans to treat patients. In consultation with the
physician(s) and care team members, Nurse Practitioners and
Physician Assistants develop plans for further evaluation of health
needs, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate
treatment for problems within the scope of the provider’s practice
as designated by protocols and identifies problems outside of their
scope of practice and refers appropriately to physicians.
Qualifications Requirements: Licensure: Unrestricted State of
Connecticut Medical License (or eligible) Training: APRN (Family,
Adult/Geri Nurse Practitioner) or PA Experience: At least two years
of post-graduate experience as a Nurse Practitioners (APRN) or
Physician Assistants (PA) New grads with relevant nursing
experience are welcome to apply!
