• Job Details
• Registered Nurse - Hospice position available.
• Performs Patient assessments: Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals.
• Ensures appropriate utilization of services: Maintains visit productivity standards.
• Completes discharge assessments: Performs laptop transfer of data twice daily.
• Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary.
• Assumes responsibility for Case Management: Develops/implements/evaluates plan of care.
• Maintains safe environment for patients and co-workers: Follows infection control measures.
• Attends monthly staff meeting: Participates in PI meetings and activities/committees as requested.
  • Job Requirements
  • Must have 2-3 years as RN in home hospice environment.
  • Must have valid New Jersey driver's license.
  • BLS - required.
